Division Change Request Form *- Parents or guardians requesting that their player Play Up or Play Down within a division of Holbrook Little League are required to complete this form. All requests are reviewed by the leagues Player Agent and will be approved or denied by the Executive Board of Holbrook Little League. Player safety and creating a level playing field are the determining factors of moving a player up or down.
